What to include
- The URL of the track or album you're reporting.

- Your full name, organization (if any), email, and address.
- A statement that you have a good-faith belief that the use of the material is not authorized by the copyright owner, its agent, or the law.
- A statement under penalty of perjury that the information is accurate and you are authorized to act on behalf of the rights holder.
- Your physical or electronic signature.
